| APPTTYPE | Type of the appointment |

| APPTTIME | Appointment time |
| ACCTNO | Patient account number |
| FNAME | Patient first name |
| LNAME | Patient last name |
| DOB | Patient date of birth |
| SEX | Patient gender information |
| WANTTEXT | Whether patient is availing the option to get text confirmation |
| CELLPHONE | Patient cell phone number |
| SENTTEXT | Reminder sent |
| SENTTEXTFAILED | Reason if sent text failed |
| CONFIRMBYTEXT | Appointment confirmed by text |
| RESCHEDULEREQUEST | Appointment reschedule request |
| CONFIRMED | Appointment confirmed |
| NOSHOW | Patient no show for the scheduled appointment |
