3. Report Filters & Parameters
| Filter Name | Description |
| Split by Master Physician | Splits the results by master physician |
| Split by Procedure Code | Splits the results by procedure code |
| Split by Facility | Splits the results by facility |
| Split by patient zip codes | Splits the results by patient zip codes |
| Hide Patient Names | Hides the name of the patient in the report |
| Include Patients with no referral sources tied | Shows the patients with no referral source tied |
| Show Insurance | Shows the Insurance carrier name |
| Select Location | Restricts results to one or all locations |
| Select service date span | Restricts results to service date range |
| Only include if first service was in the date span | Only includes the patients with first service during the selected date range |
| Select Patient Created date span | Restricts results to patients created within the selected date range |
| Referral Source | Restricts results to one or more referral sources |

| Total Charges | Total billed amount |
| Total Payments | Total payments received |

Grouped by Location, sorted by referral source
